The Oregon Perinatal Collaborative advocates for improved maternal and neonatal outcomes through collaboration, implementation of evidence-based practice, and policy change throughout the state of Oregon. This survey is part of our efforts to improve the safety and experience of community birth to hospital transfers.
The survey will take about 10-20 minutes to complete. Your responses are voluntary and will be confidential. There are opportunities at the end of the survey to share any additional comments you would like. We do not collect identifying information such as name, date of birth, or medical record number. Responses will be grouped together and analyzed as a group.
