Facility Observation Checklist – Private, Public, Canadian

1.Client rights and responsibilities are posted in the reception area
2.Smoking is prohibited in service delivery and administrative environments

For public agencies, smoking is also prohibited within 20 feet of entrances, operable windows, and the ventilation systems of enclosed areas
3.The work environment is conducive to providing services in a private and confidential manner
