Only the COMPLETE Form/Level 8 is eligible for program funding.

IMPORTANT: This order form ensures you will be eligible for ADE supported funds for your 2ND GRADE students. Licenses will not be added to your account immediately. After 2nd grade testing is complete, the test event is closed, and n-counts are confirmed, your license balance will be replenished. During this period, your balance may temporarily go negative. You will not be charged for any 2nd grade license overages.This program will not fund other grades or Screener Forms. Other testing will be invoiced to your school/district.
