Introduction

Your Oregon Nurses Association (ONA)/Samaritan Lebanon Community Hospital (SLCH) Bargaining Team will use the results of this survey to formulate contract proposals. It is extremely important that we hear your opinion on issues that will be considered in the negotiations. Each section of the survey has a different topic, and in it, you will assess things such as the current contract, work environment and safety, nurse staffing, quality of care and your intent to stay at SLCH. Please complete all parts of the survey. Space is provided at the end of the survey for additional comments should you wish to make them. Your responses will be kept confidential and your name will never be used in connection with your responses.

Instructions: PLEASE NOTE

1. Complete the survey in one sitting. We estimate that it will take about 25 - 30 minutes to complete.
2. You may move back and forth between parts of the survey. However, when you have pressed "done" at the end, you will not be able to re-enter the survey.
3. If a question has an asterisk (*), it means the question is very important and you will need to answer it before being able to move forward in the survey.
4. You should not work on the survey while on work time.
