Your Healthcare Survey

The Ministry of Health is asking healthcare providers to develop a new system of care focused on improving how you receive timely and coordinated services. Community agencies, including those with a focus on caring for clients with chronic illnesses and mental health conditions, family doctor offices, home care and your hospital are working together to develop a plan. To help create this plan, we’d appreciate hearing from you on these important questions. 

 Please do not include any personal identifying information on any responses as this survey is meant to be anonymous.
1. What would a positive healthcare experience look like for you?
2. What would be helpful as you move through different steps of the healthcare system?  For example, family Doctor, hospital, home care and community care.
3. How should healthcare providers help you feel more confident managing your health?
4. How should healthcare providers include the wellbeing of family/friends in your care?
5. How might the use of technology (e.g. electronic appointment booking, registration, sharing of information and virtual care etc.) improve your healthcare experience?
