2019 Health Impact Award Nomination

The Oregon Health Forum annually presents its Impact Award to a person contributing to their community’s health in innovative and sustainable ways. The award honors those who create healthier communities by innovating, inspiring, and implementing.

The 2019 award will recognize an individual who is making a meaningful difference through collaboration with partners to address the social determinants of health. It will be presented at the OHF “Partners for Health” Dinner on September 10, 2019
