The Ohio Choose Safe Places (OCSP) pilot program is designed to help early child care and education center operators protect staff and children from harmful environmental hazards and chemical toxins. To develop this program, ODH has worked closely with the Ohio Department of Job and Family Services (ODJFS), Ohio Department of Education (ODE), Ohio Environmental Protection Agency (OEPA), and the federal Agency for Toxic Substances and Disease Registry (ATSDR).
OCSP, which will operate as a pilot program from 04/01/2019 - 03/31/2020, is for potential child care center operator applicants who are seeking a first-time license, and for current child care center operators seeking a license renewal as part of a change of ownership or a license renewal as part of a change of location.
The purpose of this questionnaire is to encourage potential and current child care center operators to consider safe siting decisions they may not have thought of previously. This questionnaire will focus on issues centered on OCSP's four key elements, which are:
Past use of the site;
Current neighboring businesses;
Natural contaminants; and
Drinking water.
This questionnaire has 8 multiple choice questions and one question requesting contact information.
