2027-2030 State Plan on Aging: Statewide Needs Assessment Survey

Introduction

Welcome to the Ohio Department of Aging (ODA)’s statewide needs assessment survey. ODA is conducting this survey to better understand what matters most to older Ohioans, adults with disabilities, and their families and caregivers. Your feedback and opinions will be used to inform the creation of Ohio’s 2027-2030 State Plan on Aging. This four-year State Plan will guide programs and services for Ohio’s older adults, adults with disabilities, and their families and caregivers across the state.

This survey is designed to allow you to answer anonymously and consists of easy to answer multiple-choice questions. It should take you only 10-15 minutes to complete. We value your perspective and greatly appreciate your participation.
