You were selected for the Oregon Department of Agriculture customer satisfaction survey. We are surveying customers who have had contact with us between July 1 and September 30, 2013.

The Oregon Department of Agriculture appreciates the opportunity to serve you. We would like to consistently provide quality customer service. It is important for us to know how we have done in serving your needs so that we may continue to do a better job. This study is also part of an effort by Governor John Kitzhaber to enhance government customer service.

The survey will only take 5 to 10 minutes to complete. Responses are confidential.
