Introduction

Olympic Community of Health (OCH) is a non-profit and one of nine accountable communities of health in Washington State. OCH brings together partners and representatives from a variety of sectors and tribes, working together to improve population health in the Olympic region, comprised of Clallam, Jefferson and Kitsap counties.

OCH is currently conducting an environmental scan focused on Social Determinants of Health (SDOH) negatively impacting health within the Olympic region. Addressing these SDOH is a key priority of our collaborative work and of Medicaid Transformation. One aspect of this research is conducting this survey with OCH partners to gain your perspectives on the topic. The survey aims to better understand:
  • the social conditions that are negatively impacting health,
  • the top social needs in the region,
  • the current activity and opportunities for intervention and collaboration
We originally planned on conducting the assessment during in-person site visits. Given the COVID-19 pandemic, we shortened the assessment and adapted it to include some questions about how social needs are being further impacted by COVID-19.

Staff most familiar with your work around SDOH and the common needs of your community should complete the assessment; it will take at most 30 minutes to complete. Your responses will remain confidential. There are no right or wrong answers. We are interested in learning from your experience, and your honest responses are truly valuable. The COVID-19 pandemic is shining a light on and will most likely worsen existing social and health inequities. As we work to get through this current moment, OCH would like to keep a focus on social needs and the social conditions that contribute to poor health. Vision for the Future
This final set of questions focus on your vision for the future and where to concentrate efforts to address social conditions that contribute to poor health.  During the COVID-19 pandemic our current reality is dissolving and society is being altered.  Inequities will certainly widen, and negative health outcomes increase.  At the same time, we have an opportunity for social change to lessen the negative consequences. As we ask ourselves how we will overcome the immediate threat, let’s also consider what kind of world we want to live in once the storm passes.

Thank you for taking the time to complete this survey and share your insight.  Once partners take the assessment, we will compile and analyze results. We’ll then schedule some small virtual groups to discuss and will eventually reschedule a regional in-person convening where we’ll identify strategies to address social conditions that negatively impact health and advance health for all.
