This survey was developed by OCD Ottawa (a peer-based support network).

We seek to build a picture of the services accessed by people living with OCD, from
the perspective of those in a supportive or care-giving position (parents, partners, other family members, friends).  

Information collected will be used by OCD Ottawa to advocate for adequate treatment and services for individuals with OCD, and their family members/supporters.  Your feedback will also help to identify areas for education and outreach. 

A summary of the survey results will be made available on the OCD Ottawa website.

Your participation in this survey is voluntary. All information shared in the survey will be treated confidentially. We do not ask for personal identifiers (name, address, contact details, etc.) and we cannot link responses with individual respondents. The results will be reported at an aggregated level and conclusions will be formulated for groups and subgroups of family members. Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password protected electronic format. Survey Monkey does not collect identifying information such as your email address, or IP address. Therefore, your responses will remain anonymous.
