I, as the parent/legal guardian of the participant, understand and acknowledge that participation in OCCSC Summer Camp activities involves certain inherent risks. I hereby consent to my child’s participation in all scheduled activities and agree that Ottawa Chinese Community Service Centre (OCCSC), its employees, volunteers, and representatives will take all reasonable precautions to ensure participant safety.

In consideration of my child being permitted to participate, I agree to assume all risks associated with participation and hereby release, waive, and hold harmless OCCSC, its staff, volunteers, and partners from any and all claims, liabilities, damages, or expenses arising from or related to my child’s participation in the program, except where caused by gross negligence or willful misconduct.

I confirm that the information provided regarding my child’s health and needs is accurate to the best of my knowledge, and I agree to inform OCCSC of any changes prior to or during the program.
