Anonymity and Confidentiality Consent Form

You are invited to participate in a web-based online survey on market research and ASWCO program and service evaluation. It should take approximately 15-20 minutes to complete. This survey has been created to discover the level of physical activity, sports of interest, mental health and demographics of our Aboriginal youth living in Ontario. 

Our goal is to measure ASWCO’s effectiveness in providing:
  1. A holistic approach to physical cultural practices;
  2. Strong family and community bonds;
  3. A willingness to draw upon both culturally derived and mainstream values, physical practices and delivery systems; and
  4. Encouraging a commitment to self-determination by example or workshops

CONFIDENTIALITY CONSENT FORM 

PARTICIPATION
Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without penalty. You are free to decline to answer any particular question you do not wish to answer for any reason.

BENEFITS
You will receive no direct benefits from participating in this survey. However, your responses may help us learn more about the quality and effectiveness of our programs and services, and gain insight on new opportunities in your community.

RISKS
There are no foreseeable risks involved in participating in this study other than those encountered in day-to-day life.

There is the risk that you may find some of the questions to be sensitive.
OR
There is the risk that some questions may cause emotional discomfort.
OR
Some of the survey questions ask about your emotional state, perceived financial situation and may be distressing to you as you think about your experiences.

The possible risks or discomforts of the study are minimal. You may feel a little uncomfortable/embarrassed/sad/tired answering personal/sensitive/many survey questions.

CONFIDENTIALITY
Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password protected electronic format. Survey Monkey does not collect identifying information such as your name, email address, or IP address. Therefore, your responses will remain anonymous. No one will be able to identify you or your answers, and no one will know whether or not you participated in the study.

CONTEST
If you choose to provide contact information such as your first name, last name or email address, for our CONTEST your survey responses may no longer be anonymous to the researcher. However, no names or identifying information would be included in any publications or presentations based on these data, and your responses to this survey will remain confidential.]
