Grant Requirements:
- Four (4) new members in current Rotary year (minimum)
- Four (4) of those members must remain members for at least four (4) months of the current Rotary year
- Five (5) $1000 grants will be awarded
- This form must be submitted by May 31st of the current Rotary year
- Funds will be sent during June of the current Rotary year
- When funds are used or project is complete, final report must be given to District 6250
