The Nantucket Health & Human Services Department invites you to complete a survey about human services on island. These services are designed to help individuals improve their lives and achieve self-sufficiency by addressing basic needs of all members of our community.

This survey is part of a comprehensive Human Services Community Needs Assessment aimed at helping the Town and County of Nantucket develop strategies that align human services with the needs of our community whether you are a full-time resident, a seasonal worker or a commuting worker.

Your responses will remain confidential, and the information you provide will be used to gain insights into how community members access human services. This data will assist us in creating strategies to effectively meet all members of our community. The survey will take approximately 5 minutes to complete.
