Dear Nantucket Resident,

The Nantucket Health & Human Services Department invites you to complete a survey about human services on our island. These services are designed to help individuals and families improve their lives and achieve self-sufficiency by addressing all residents’ basic needs.

This survey is part of a comprehensive Human Services Community Needs Assessment aimed at helping the Town and County of Nantucket develop strategies that align human services with the needs of our community.

Your responses will remain confidential, and the information you provide will be used to gain insight into how residents access human services. This data will assist us in creating strategies to effectively meet community needs. The survey will take approximately 10-15 minutes to complete.
