Welcome to the Human Behaviour Questionnaire

The aim of this study is to gather New Zealand information for a range of health outcome questionnaires. The data gathered will be compared to that which has been previously collected from individuals (and family members) who have experienced Traumatic Brain Injury so that we can find out more about the long-term effects of these injuries.

You are eligible to take part if you live in New Zealand, are over 16 years of age and have not had a brain injury or concussion in the last 12months.

You are being asked to complete an online questionnaire, at your convenience, which will take approximately 25 minutes.

The questions focus on physical and mental health, community involvement and substance use. All the information you provide is anonymous and not linked to your name.

You can withdraw from the study prior to submitting your responses simply by closing your internet browser. As a thank you for taking part in the study you can choose to enter a draw for a $250 Prezzy Card.

All the data will be stored on a password protected computer. Names will not be linked to the data, so no-one will be able to identify your responses.

The findings from the study will be written up as part of a Masters thesis, peer-reviewed journal articles and presented at conferences.
