Ketchikan Survey: Attitudes About COVID-19 Vaccine

The Ketchikan Public Health Center has created this survey to understand attitudes and feelings surrounding COVID-19 vaccines in parents and families.

This survey's purpose is to determine and anticipate vaccine demand if/when COVID vaccines become available for 5-11 year olds. Other goals include include: understanding your concerns about COVID vaccine and barriers families might face to getting vaccinated. We hope to improve our processes and address barriers.

We will not ask your name or any personal identifiers, and all information in this survey will be held confidential. 

The survey consists of 18 questions and should take about 5 minutes.
