About this Survey

Multiple Sclerosis New Zealand would like to hear more about you, your life with MS and how you experience the health services in NZ.

We want you to help inform us about what the needs are within the MS Community in NZ and how we can work and advocate to help improve services.

Can you help us shape a fair future?

MS specialist services both in the community through Regional MS Societies and in the DHB setting do a brilliant job. But we know that in many areas they are under-resourced, dealing with increasing pressures and facing difficult new challenges.

We want to make sure no one living with MS misses out on the specialist care they need. However, we need your help to understand this.

If you have five minutes, please share your views and experiences on MS services in our short survey.

By completing this survey, you're helping us to build up a clear picture of the state of MS care in NZ today, and allowing us to identify the key issues that need addressing.

You are welcome to stay anonymous in this survey or you can choose to share your contact details with us so we can send you future surveys to help us gather information that we can use to advocate for changes or improve MS services in the community. If you share your details with us you will be put in the draw to win a copy of the 2015 Esme Tombleson Person with MS of the Year, Sarah O'Neil's book 'The Good Life: four glorious seasons in my country garden.' You must complete this survey by Friday 6th May 2016 to go in the draw.

If you've already completed the survey, having seen it on our Facebook, received it from us by email or through your Regional Society thank you very much. Your support will make a real difference to the future of MS services.
