Thank you for taking the time to complete this survey by the Wyoming Department of Agriculture's (WDA) Analytical Services Laboratory (ASL).  As part of our commitment to quality, the ASL is seeking your feedback on issues relating to data quality and overall customer service.

Laboratory management will evaluate results of the survey.  The results of the survey may be shared with WDA Division managers and appropriate ASL staff once complete.

The  goal of  this survey is to provide the information needed for the continual improvement of the services provided by the laboratory.  This survey should only take a few minutes of your time.
