Consent Form for Research

Invitation to be Part of a Research Study
You are invited to be part of a research study. This consent form will help you choose whether or not to participate in the study. Feel free to ask if anything is not clear in this consent form.

 

 

Why is this study being done?
 

The purpose of this study is to assess if a different type of diversity training is more effective than previous types of trainings.

 

What will happen if I take part in this research study?
If you agree to take part in this study, you will be asked you will be asked to attend a diversity training session and then meet in established groups seven times after the session. You will take a survey three times: before the training, right after the training and six months after the training. 

 

How long will I be in this study and how many people will be in the study?
Participation in this study will last anywhere from 2 to 4 months with a final followup questionnaire six months after the training. About 50 to 75 of subjects will take part in this research study.

 

What are the risks of taking part in this research study?
There are some risks you might experience from being in this study. They are potential discomfort in relating your racial attitudes and in conversations with others in your group.  

 

 

Are there any benefits from being in this research study?
You might benefit from being in this study because of the diversity training you receive from the study.

 

How Will You Protect my Information?
 

To allow the principle researcher to track the pre-test, post-test and follow-up test answers the researcher will keep track of the emails and key identifying information of the participants. That information will be destroyed soon after the completion of the last survey and before any public reporting of the results.  

 

 

Your Participation in this Study is Voluntary
Taking part in this study is your choice.  You are free not to take part or to withdraw at any time for any reason.  No matter what you decide, there will be no penalty or loss of benefit to which you are entitled.  If you decide to withdraw from this study, the information that you have already provided will be kept confidential. You cannot withdraw information collected prior to your withdrawal.

 
Consent to Use Data for Future Research

I agree that my information may be shared with other researchers for future research studies that may be similar to this study or may be completely different. The information shared with other researchers will not include any information that can directly identify me. Researchers will not contact me for additional permission to use this information.
