IHE Outreach Survey

Thank you for agreeing to take the New York State Public Health Corps (NYSPHC) Institution for Higher Education (IHE) Survey. This survey will be used to collect information about Colleges and Universities across NYS and inform the NYSPHC's outreach activities and solicit interest in the Public Health Corps Working Group and Mentorship. If there are questions that are not applicable to your Institution or job title, please feel free to “skip” the question or choose “N/A.” The following survey should take approximately 10 minutes to complete.
