The purpose of this anonymous survey is to serve as a healthy evaluation of 5C, meeting effectiveness, and an open communication to improvement and change. This is your opportunity to share your thoughts, ideas, comments, or concerns in regards to the 5C Coalition. All feedback will be reviewed regularly at monthly meetings.
