Your facility has been assigned to the 2020 Decreasing LTC QIA: Steering Committee
This QIA is designed to decrease the rate of dialysis patients utilizing a central venous catheter for greater than ninety days. We will accomplish this years outlined CMS goal through sharing best practices, improving processes used for permanent access placement and/or intervention, long-term catheter education and awareness, improved communication with local healthcare providers, and directing interventions toward patients to allow them the ability to impact their own care.
This quality improvement activity is a CMS initiative; therefore, your active participation is mandatory. We hope that you find this project to be a useful addition to your ongoing quality improvement measures and we appreciate your efforts toward making the project successful.
This QIA will involve several components, including written and verbal participation in the Qsource ESRD Strategies taskforce (termed Vascular Access Management Learning and Action Network or VAM LAN), contribution to the development of staff and patient dialysis access education, and sharing strategies to improve communication with local healthcare providers. Please consider these when selecting your project lead and co-lead. Please Note-To fulfill the components of the 2020 Decreasing LTC Steering Committee, one of your Project Lead designations must be a Patient Care Technician or CCHT.
