LARCS Program Application

This Application form is for use by a private entity requesting approval from the LARCS Board of Directors to hold a program on LARMAC owned property as a LARCS Program.  This form must be completed and submitted for review and approval in order to become an official LARCS program.  Program Applications must be submitted a minimum of (4) months in advance of the proposed program start date.

Please Note: If you are proposing more than one program then you must complete this form for each program you are proposing.
