To whom it may concern,

Thank you for your interest in this investigation of supplement use among athletes. If you are aged between 18- 45 years and participating in sports training on a regular basis, please continue reading.

This questionnaire forms part of a PhD programme investigating peri-training nutrition (PTN) and athletes (e.g. amateur, recreational or elite) are the source of this information.

The aim of this questionnaire is to learn more about supplement consumption. This includes supplement type, time, dose, frequency as well as rationale of its intake across various sports disciplines and the time of competitive season. We are interested in your opinion about motives of supplements consumption and current practices. However if you are an athlete who does not consume any supplements you are also welcome to participate in this study. The reasons why you do not take supplements are also of interest. The information we collect from this questionnaire will allow performance nutrition practitioners to identify opportunities for further education in the pursuit of optimal PTN - the eating occasions before, during and after athletic training, to maximize training adaptations.

This questionnaire is anonymous and your answers are confidential. The data will be anonymously used in report and published output.

The data collected from this questionnaire will be stored securely and anonymously for 7 years and after that time data will be destroyed. There are no risks to you associated with participation in this study. 

The completion of questionnaire will take approximately 20 minutes of your time. Please be honest as possible!
