Hello Oregon School Food Authority!
We want to hear how the ODE Farm to School Procurement Grant for School Year 2016-2017 made an impact on your Oregon purchases and farm to school education in your district.
The final report is mandatory, and required of all schools that opt in to the ODE Farm to School Procurement Grant for purchases of Oregon food.
The survey should take about 1 hour to complete. It may be helpful for you to review your baseline data, as well as your progress report data (for year one opt in districts only).
