NSW Young Person Survey

CREATE Foundation is the peak consumer body representing the voices of children and young people in out of home care in Australia.  We are interested in hearing your views about Information Sharing and how best we, as an organisation, can support you.

Please take a few minutes to help us be the best we can be, by filling out this survey.  The information you provide will be used to advocate to government so that we are able to promote the voices of children and young people more effectively.

Survey closes 27 November.

The first 30 surveys completed will receive a $20 Coles/Myer gift card!

IT IS important for children and young people in out-of-home care to know about what the peak consumer body for children and young people (CREATE Foundation) does.
IT DOES NOT matter whether children and young people in out-of-home care have a say in decisions affecting them.
Children and young people in out-of-home care SHOULD NOT have access to someone independent of governments and placement agencies to make sure their voices are heard.
FACS SHOULD provide CREATE Foundation with contact information about children and young people in care to help its staff advocate on behalf of those in care.
When children and young people are brought into care, they SHOULD AUTOMATICALLY have the right to be connected to the peak consumer body for children and young people in care (CREATE Foundation).
