The P.I.E.C.E.S. Learning and Development Model is a holistic person and care partner-directed TEAM approach to shared solution finding, early detection of and response to change in the person, enhanced shared care planning and ongoing monitoring. The P.I.E.C.E.S. Leadership Performance Improvement Program has been designed recognizing the role of leadership in the development of high performing TEAMs with a shared commitment to collaborative best practices and ongoing learning.

The purpose of the P.I.E.C.E.S. Leadership Performance Improvement Program is to:
* Provide opportunity for shared learning and knowledge exchange.
* Provide an understanding of the current P.I.E.C.E.S. TEAM approach using the P.I.E.C.E.S. 3-Question Template.
* Explore the work environment and individual factors to support learning and performance improvement.
* Provide opportunity for 'possibility conversations' and identifying priorities and support required for enhancing person and care partner-directed care.
