If selected, I will appear and present the above-described concurrent session at this conference. I will provide (50) copies of handouts for participants and a digital copy of handouts/slides.

The conference planners will correspond with the primary presenter only. It is the responsibility of the primary presenter to notify each co-presenter of pertinent information concerning the presentation.

ALL EXPENSES ARE MY RESPONSIBILITY except for the cost of registration which will be deferred for the lead presenter only.

I understand an honorarium of $300 will be awarded which includes admittance to the conference as well as continental breakfast, lunch and snacks. If a presenter IS SELECTED for a SECOND SESSION, there will be an additional $100 honorarium awarded.

Release of my image and contact information (address, phone, fax, email) for use in the conference program and other materials is authorized.
