District Equity and Climate Survey

Purpose
Your completion of this survey will help your school and the District determine how successful our efforts are to promote positive culture and climate. It is our responsibility to ensure that all students feel safe at school and are free from harassment and bullying, especially as it relates to race, color, or national origin. The data will be used for planning and training staff and students. It will also be used for evaluating if our procedures and policies are effectively implemented to ensure that our schools and district have a positive culture and climate.

Confidentiality Statement
All responses are strictly confidential. No personal identifying information is collected, and while we do ask for some demographic information, this information will not be used to identify you.

Definitions
Please read these definitions to understand the important behaviors this survey is collecting data about.

Bullying. Behavior or a pattern of behavior demonstrated when a person repeatedly uses power to intentionally humiliate, control, or inflict harm. These behaviors can include verbal, physical, or written conduct and/or intentional electronic communication (text message, email, and social media (Facebook/Instagram/Twitter/Snapchat, etc.) against one or more students.

Harassment. Includes actual or perceived negative actions, which offend, ridicule, or demean another individual with regard to race, national origin, color, sex, sexual orientation, gender identity, religion, physical attribute, socio-economic status, familial status, and/or physical or mental ability/disability.

Equity of Academic Opportunities. Students experience equity of academic opportunities when they have all of the necessary individual support that they need to participate and excel in all academic activities.

Structure
You will first be asked questions about bullying and harassment, discipline, school climate, and equity of academic opportunities at your school. We will then ask for demographic information about you (grade(s) taught, gender, race, etc.). The survey should take no longer than 10-15 minutes to complete. Responses will not be saved if the document is closed before submitting the answers.
