Welcome to the Neighborhood Services Survey.

The City of Lima is seeking community input through the Neighborhood Services survey to help identify community needs in each neighborhood area as we prepare for the 2016-17 annual action plan, the 2nd year of the current 2015-19 Five Year Consolidated Plan. The Lima Consolidated Plan is the City's strategic plan required  for HUD federal funds. This survey is also useful in directing city program and service planning. The survey is confidential and the general information collected may be required for some funding programs.

 

There are a variety of questions about neighborhoods, with several ways to respond. Please answer all questions, and add comments if space is available. The results from this survey will help us determine what programs and services are needed in each neighborhood area.
