Please fill out the questions below for the nominated program / person. The experienced panel of judges will review and score the information, and will not accept additional information, brochures, etc. on the program, so do your best to describe the worthiness of the nominee clearly and succinctly.
The completed form is DUE 5 pm EASTERN TIME, August 21, 2015. We expect / prefer a representative from the winners to be at the award ceremony at the Resource Recycling Conference September 28-30th in Indianapolis.
Judges will review the submittals and contact about any follow-up questions between August 22 and September 4th; we intend to notify the winners by September 7.
Note that you will need to PROVIDE 2-3 LETTERS OF REFERENCE in support of the award. We ask for the reference names in this document; you will have until August 21 to forward PDFs of the letters via email using the instructions at the end of this form.
