Mid Year Performance Evaluation for Employers

This performance review provides us with the information we need to help participants become better employees. We ask that you evaluate the participant’s performance accurately and objectively.

Throughout this survey, you will use a 5-point scale to evaluate the participant's performance. This scale allows you to assign a neutral score (3) to participants who are meeting expectations. Participants deserve a ”3” rating when they meet the expectations of their role. Under this rating system, a score of 3 is good!
