Support Recognition Nomination Form

This program is designed to recognize outstanding performance by any associate in a non-agent/non-management role at ADTRAV and Adventure Travel. Your nomination will be considered for the quarterly Super Star Support award as well as the annual Super Star Support Award given at our annual meeting in January. Please honestly rate your nominee below. You must include comments on why you feel this person is deserving of this award. A point system will be used in determining the quarterly and annual winners. In the event of a tie the Support Recognition Committee will review all nominations and vote for a winner. Nominees names will be withheld and will remain anonymous until after the voting has taken place.
