About the project
WA Primary Health Alliance is currently working with Impact Co, an external consultancy firm, to conduct a needs assessment to understand how primary health care services in the after hours period, and for multicultural communities and those experiencing, or at risk of homelessness are delivered across WA.
About this survey
You are invited to complete this survey because you are a service provider who operates, or may have experiences relevant to one or more of these PHN program areas:
  • Primary health services operating during the after hours period
  • Programs or services for multicultural communities
  • Programs or servicers for people experiencing, or at risk of, homelessness
The survey aims to identify the:
  • current state of primary care services that operate in these three areas,
  • current and unmet need across WA; and
  • opportunities to enhance primary care services to improve health outcomes.
This survey should take 10 minutes to complete.
Your information and privacy
The information we gather from this survey will provide valuable insights for the needs assessment. It will provide information on the barriers, service gaps and challenges in the provision and access of primary health care services by multicultural communities and individuals experiencing homelessness, and during the after-hours period in WA.

Any information collected will not be identifiable, and it is optional to complete this survey. Please note that you will not be eligible for reimbursement for this survey. However, your contribution will help inform decisions made by WA Primary Health Alliance on the future commissioning of primary health services.
