Forest Stewardship Council® Certification Annual Renewal Questions

The following questions are part of the annual FSC certified member check-in process with NNRG. These answers will be compiled and used to complete the annual FSC audit for our group certificate. Answers may be left blank if there are no updates.

1.What is the name of your forest?(Required.)
2.Have there been any changes in property information (ownership, easements, current use taxation)?
3.Have there been any major changes in management objectives?
If so, why?

Are you meeting your stated management objectives?
4.When did you last update your forest management plan?
(Please note, management plans must be updated every 10 years.)
5.Have you conducted a commercial harvest project since the last check-in? If so, please describe:
  • Harvest start & end dates
  • Timber harvest activity (volume, species, grade, purchaser)
  • Harvest methods
  • Acres treated
  • Stand prescription
  • Post-harvest activities
6.Have you conducted a pre-commercial thinning since last year's check-in? If so, please describe the stand, change in trees per acre, acres, and how the work was done.
7.Do you have plans for an upcoming harvest? If so, when? Methods? Acres? Treatment goals?
8.Have you done any value-added manufacturing since the last check-in? If so, please describe.
If any products have been sold as FSC certified, please share species, volume, and gross income.
9.Has there been any commercial harvest of non-timber forest products? If so, please describe the species, volume, area harvested, how you monitor the site.
10.Have you used any pesticides or other chemicals in your forest? If so, please provide the following: 
- Herbicide name
- Undiluted volume used (REQUIRED)
- Diluted volume used
- Date range of application
- Area treated
- Location
- Method of application
- Target species
- Observations of effectiveness (post-herbicide monitoring)
11.Have you used any biological control agents, genetically modified organisms, or exotic species in your forest management? If so, please describe.
12.Has there been any new road construction? If so, please describe.
13.Please describe any road maintenance and monitoring.
14.Have you had to do any bridge installation/removal? If so, please describe.
15.Have you had to do any culvert installation/removal? If so, please describe.
16.Have you had any erosion problems? If so, please describe.
17.Have you conducted any management activities next to a waterbody? If so, please describe.
18.Do you have problems with invasive species? If so, please describe which species and how you are treating them.
19.Of your 'production' forest, how much of the land is regenerated naturally, and how much is generated by replanting (in number of acres, this can be approximate)?
20.Do you have old-growth on your forest, or areas that you have classified as high conservation value forest
(HCVF)?
Please describe any changes to old-growth or HCVF.
21.Describe how you have monitored your forest in the past year and how you are updating your management records.
22.Any sightings of rare, threatened, or endangered species?
If so, have you done any work to enhance their habitat? 
Is your forest enrolled in any habitat programs with ODFW, WDFW, or USFWS? If so, which programs?
23.Are you currently enrolled in cost-share programs (NRCS EQIP or CSP, local Conservation District programs)? If so, please describe.
