Risk Assessment

Answer the following questions to the best of your knowledge.  Higher score is indicative of a higher risk of injury.

* 1. Does the athlete participate in a sport that involves jumping, pivoting, or quickly changing directions?

* 5. Has he/she had an injury to the anterior cruciate ligament (ACL) in the past?

* 6. These are flexibility questions:  Can the athlete reach their thumb to contact their forearm (on the same hand)?  Do their knees and elbows hyperextend when they straighten them all the way?

* 7. These are movement questions:  If you watch them closely as they land from a jump, do the knees move inward?  If you ask them to balance on one foot and squat, does the knee move inward?

* 8. Does the athlete demonstrate good warm-up and cool-down habits?
