Task Force Listening Session Registration

Welcome

Governor Mike DeWine established the Ohio Governor’s Nursing Home Quality and Accountability Task Force by Executive Order to study issues surrounding quality of life and quality of care in Ohio’s nursing homes.

We want to hear from you! By participating in a listening session, you can share your personal experiences, make your voice heard and help us achieve our goal of making excellence the expectation for every Ohio nursing home.

Question Title

* Your attendance at a meeting constitutes your consent to be photographed, filmed, and recorded, and to the release, publication, exhibition, reproduction of any and all media for use on websites, in social media, news and publications. By attending a meeting, you waive and release any claims you may have related to the use of the recorded media. While any personally identifying information provided would be protected, testimony provided would be subject to a public record subject to release on request by a member of the public.
