Nursing Home Satisfaction Survey Questionnaire
Purpose:
The purpose of this questionnaire is to gather information to identify how nursing homes are using resident and family satisfaction surveys to improve quality. The information gathered through this solicitation may be used to develop programs and policies aimed at improving resident and family satisfaction outcomes, and how those outcomes can be used to improve quality.
Background:
New York regulates over 620 nursing homes throughout the State, with approximately 100,000 New Yorkers residing in nursing homes. The Department of Health (Department) recognizes the importance of ensuring that all New Yorkers residing in nursing homes receive quality care.
The Department is very interested in any initiative targeted at improving nursing home quality. This questionnaire is one of the first steps in a series of projects that will seek to gain valuable input on how to improve nursing home quality across the State.
Information Request:
The Department is seeking information from nursing home providers. Of particular interest is feedback from providers that are currently or have previously undertaken quality improvement initiatives developed from the results of resident and family satisfaction surveys. Providers should submit information and data on best practices currently in use and recommendations on how to effectively and efficiently implement successful quality improvement programs. We are seeking only one response per nursing home.
Please have the Chief Information Officer (CIO), or equivalent title with decision making authority over IT matters, fill out the survey.
