2026 NHPC Community Survey

Dear Community Member,
The Northwest Hills Prevention Connection (NHPC) is conducting a brief community survey to better understand local needs and priorities. Your input will help guide efforts to support the health and well-being of our community.

The anonymous survey takes about 5 to 7 minutes to complete. All responses are confidential, and you may skip any question.
