ACHA Lifetime Achievement Award Submission Form The ACHA Lifetime Achievement Award is the highest honor that the American College of Healthcare Architects can bestow on an architect. It is conferred by the national ACHA Board of Regents in recognition of a significant body of work of influence on the theory and practice of healthcare architecture. Submissions are due by June 30, 2026.EligibilityAny architect (living, retired or deceased no longer than 5 years from the submission deadline), who has positively influenced the outcome, delivery or environment of healthcare is eligible to receive the ACHA Lifetime Achievement Award (LAA). For the purpose of the award eligibility, “architect” means any architect who is registered or licensed in their country of practice. Submission ContentPlease be sure to have all items listed below: Nominee and sponsor contact information (including name, email, firm name, phone number, street mailing address, and certification details). The sponsor can be an architect, an ACHA certificate holder, or any individual who is familiar with the candidate, their body of work and area of influence. A nomination letter not to exceed one page that summarizes the contribution made should be submitted by the nominee’s sponsor. The letter should focus on the nominee’s influence on the outcome, delivery or environment of healthcare. The letter should, contain specific reasons for support and clearly delineate how the nominee has made a difference within the profession. A brief biography not to exceed two pages. Biographies exceeding this limit will not be considered. A list of the nominee’s achievements not to exceed three pages. A list exceeding this limit will not be considered.
