This survey is intended only for manufacturers that are currently or previously have been impacted by the Federal Insecticide Fungicide Rodenticide Act (FIFRA) regulations.

NOTICE AND DISCLAIMER: All individual responses and data collected through this survey will be maintained as strictly confidential within WQA. WQA will not release your submission information publicly except in an aggregated and shielded format where at least 10 survey responses have been collected. WQA will not name you as a survey respondent nor attribute aggregated responses as including your data or the data of a manufacturer you may represent as an importer or exporter in any release of aggregated information to a third party without your express written consent. Subject to the foregoing, by participating in this survey, you have agreed to permit WQA to collect, store, and analyze your data for release in aggregated form.
