Your voice matters. This survey is dedicated to our community members and their needs. Please help strengthen our health care system not only on the clinic side, but in all areas of our community including food systems, nutrition, transportation, youth, elders, individuals with disabilities, etc. The visionary areas of the strategic plan continue to collaborate on improving these services.
