Perspectives on PVFMD
Welcome to My Survey
In the following survey, you will be asked questions related to the assessment and treatment of airway disorders caused by laryngeal obstruction (e.g., paradoxical vocal fold motion disorder, vocal cord dysfunction, irritable larynx syndrome, etc.). Inclusion criteria for participation of this study includes being a professional in one or more of the following fields: speech-language pathology, laryngology, otolaryngology, pulmonology, allergy, gastroenterology, and psychology. Participants must also be 18 years of age or above. The estimated time to complete the survey is 15-20 minutes.
Your responses are anonymous, and the only personal information collected is geographical location and work setting. Here is the link to SurveyMonkey’s privacy policy https://www.surveymonkey.com/mp/legal/privacy-policy/ to ensure that you understand the website’s privacy statement.
Benefits to future patients and society include gaining a better understanding of assessment and treatment methods of this airway issue. This study will also provide awareness for future research on the assessment and treatment of laryngeal obstruction. A risk of participation may be feeling uncomfortable due to lack of knowledge related to subject material.
Once you begin the survey, you can exit it at any time if you are no longer willing to participate. The Informed Consent form for this study is located below. Please read the form and the following statement to make an informed decision to participate in this study:
I am 18 years of age or older and I agree to voluntarily participate in the study and that my responses can be used for research purposes. By continuing with the survey, you consent to participate and for information, such as a geographical location and work setting, to be collected.
