We are asking for your help in filling out this questionnaire by January 1!  It should take less than 15 minutes to complete.

The community health improvement workgroup is developing a work plan to address four priority health issues in north-central Nebraska.  The priority issues are:

> Physical Wellness
> Aging and Related Issues (including chronic disease, safe housing, environmental concerns, etc.)
> Mental Wellness
> Substance Abuse

These priority areas were identified through the process of completing a community health needs assessment.  The workgroup is made up of members from a wide variety of organizations working together for the health and wellness of our communities throughout Antelope, Boyd, Brown, Cherry, Holt, Keya Paha, Knox, Pierce, and Rock counties.

As we develop the work plan and identify possible activities to address these health issues, it is important to be aware of what resources, programs, services, and activities currently exist.  Your organization has been identified as one who may currently be working in one of these areas.  We are asking you to fill out the attached questionnaire so we can learn about existing resources, connect people to resources, and ensure we don’t duplicate efforts.  Where possible, we would like to partner together to help support your existing programs and engage your support for any new activities we identify for implementation.

Please complete the questionnaire by January 1, 2017.  Feel free to forward on to others who provide services in these counties, as well.
