The Northeast Florida Cancer Control Collaborative is developing a toolkit to guide local healthcare organizations and community groups in establishing patient navigation programs.
Patient navigators assist cancer patients and caregivers with finding information, identifying and accessing available resources, coordinating services among multiple providers, arranging logistical assistance such as childcare and transportation, and overcoming other barriers to care.
Your responses to these questions will help us better understand the needs in Duval County and develop strategies to address them. Your participation in this survey is greatly appreciated!

Definitions, for the purposes of this survey:
Patient - Anyone who has been diagnosed with cancer, whether currently or previously in treatment.
Provider - A healthcare professional who provides cancer care or treatment services within a healthcare organization.
Caregiver - Anyone who cares for a cancer patient in the context of a personal relationship (i.e. family member, friend, relative).
