NCHS Annual Renewal Form

Welcome to a new school year!

Please take a few minutes to complete the annual renewal form to let us know your club's status and any new contact information.

**If your club was inactive and plans to reactivate this year, we ask that you distribute the Parent Letter before you begin filling out this form. (Contact nchs@namica.org if you need a copy of the Parent Letter.)

Please complete this form no later than September 30th so we can be sure to include your club in our outreach and opportunities for this year.
