Breakfast/Lunch Participation Survey for April 14-April 17

Greetings,

I apologize for continually changing the format of the survey. We are trying to find the right option to give us more accurate numbers and to ensure those who complete the survey get the meals they requested. This week, we will be serving some hot food options. If you pick up the meals in Geddes, heating instructions will be provided.

If you forget to complete the survey or can't remember which days you have selected, call my office at 337-3391. If you complete the survey and cannot make it to pick up the meals, please let us know you won't be coming.
