The Community Health Needs Assessment (CHNA) is data compiled by health departments and communities within Omaha Counties – Douglas, Pottawattamie, Cass, and Sarpy. It is used to identify, “health status, behaviors, and needs of the Omaha metropolitan area” and will be used to guide efforts to improve health and wellness.
